Please fill the following form to request a new ID Badge: https://bit.ly/new-badge
After fill the form, please download and fill the confirmation document (download here), and send it to the ID Badge team for ID Badge Issuance.
 

After fill the first form, please proceed to fill the Preliminary Additional Attachment Form through this link: https://bit.ly/preliminary-attachment
And then, please contact the HSE team to get ID Badge Sticker, and the ID Badge will be acitvated for 1 year.

Setelah mengisi form, silakan unduh dan lengkapi dokumen konfirmasi (unduh di sini), kemudian kirimkan kepada tim ID Badge untuk konfirmasi pencetakan ID Badge.
 

Setelah mengisi form yang pertama, silakan lanjut mengisi Form kelengkapan dokumen-dokumen tambahan pada link berikut: https://bit.ly/preliminary-attachment
Setelah pengisian form tersebut, silakan mengubungi tim HSE untuk pengambilan sticker, dan ID Badge akan diaktifkan selama setahun.
